60 minute Initial Consultation & Assessment
During your first session, we will discuss your history and current food concerns and behaviors. This will build a foundation for us to develop treatment goals and identify what you hope to accomplish with nutrition therapy.

60 minute In-depth Follow-up Nutrition Therapy
Extended sessions are available for in-depth nutrition counseling and support as you progress towards identified treatment goals. Sessions in alternate locations and settings may also be scheduled depending on individual client needs.

30 minute Standard Follow-up Nutrition Therapy
Standard sessions are available for on-going support and nutrition counseling, depending on individual client needs.

Out-of-Network Billing & Superbills
If we are not contracted with your insurance company, you may have an out-of-network benefit. Contact your insurance company prior to your appointment to determine if you are eligible for reimbursement. Click here for questions you should ask. As a courtesy, we can submit the claim directly to your insurance, or provide you with a superbill that you can submit for reimbursement. You will be charged the insurance-billing rate at the time of service.
Self-Pay
Payment is due at time of services for self-pay at a discounted “prompt pay” rate. For your convenience, Positive Change Nutrition accepts all major credit cards, cash, FSA, HSA, and Venmo. Self-pay is NOT eligible for OON billing or superbills.

- Is nutrition counseling covered? (ask about procedure codes 97802 and 97803. If these
codes are not covered or limited, ask about codes 99401, 99402, 99403 and 99404). - Will my diagnosis be covered? (ask about nutrition diagnosis codes Z71.3 and Z72.4, if these diagnoses aren’t covered, ask if a medical diagnosis is required, ex. eating disorder, diabetes, etc.)
